This is a known issue.
To correct the issue on each project experiencing this issue all you have to do is to perform an upgrade process in Site Administrator.
NOTE QC must be at Patch 12 or above before performing these steps.
1) Login to Site Administrator
2) Highlight the project
3) Right Click and select Upgrade Project from the menu.
4) Answer the Prompts
a) Yes to Continue you are familiar
b) Yes to Deactivate the project
c) OK to Disconnect the users
d) No, If prompted to move the project to the database
e) Yes, if prompted already at the latest version upgrade anyway
f) Yes, to reactive the project
