Updating Server for ALM 12.50

Currently, we are on HP ALM QC v 11.52 and are looking to upgrade to version 12.50 within the next few months. Is there an HP or Orasi checklist that we could use to document the requirements, existing server configuration, and upgrade process to a newer server. Any information that you can forward would be greatly appreciated. Thanks in advance.
