Upgrading UFT breaks Java Add-in for Oracle Forms?

We upgraded our test developer’s UFT box from 12.02 to 15.0.2 and now the Java Add-in no longer works with our application made with Oracle Forms. Is this something that can be fixed?

Here are a few things to try: *Uninstall and reinstall the Java Add-in by performing a "Change" on the UFT installation and removing the Java Add-in, then another Change and select it again. *If that doesn't work, ask your support representative for either the manual steps for a clean uninstall of UFT or for the UFT Uninstall Tool.
