What you are experiencing sounds like a known issue documented within HP (QCCR1J92712 documents this issue). As per the HP recommendation in the QCCR:

''We have suggested customer to remove ''/'' character with some other ..say ''-'' ''

I would check that you may be using this special reserved characters within your generation and if so replace them as suggested. This is a known issue noted since the upgrade and implementation of ALM12.53.

If this recommendation does not apply to your situation, then i would recommend that you open a case with your support to address your individual situation and assist in getting a resolution to your issue.
