When creating Defect, Bug_New is triggered several times in ALM 12.55?

Hello,
We have discovered that when we are trying to create a new defect from any module the "Bug_New" action is triggered several times. We are on ALM 12.55 and have never had this issue before. Is this a known issue or is there a known bug relating to this issue?

Hello HomerJ,
I have seen this noted within the knowledge base of Microfocus. QCCR1J99891 covers the issue and it is a known bug that has been fixed within patch 4. I order to correct this issue, please install patch 4 or later on your ALM 12.55 application server to correct the issue.
