My .NET addin for QTP 9.5 is taking long time to load. When the addin does finally load, the addin is does not work.

I have recorded a script on a web page and the page contains a weblist but when I play the script back it does not work. The recording captured the event of setting the value of the web list but will not set it. Has anybody else seen this issue?

My QTP 10 main menu item has disappeared. For example when I try to open a test (File > Open) there is no option for Open. All of the sub selections are missing.
