This is a know issue for all projects created before QCv92 patch 12. To correct the issue: 1) Install the latest patch (After Patch 12) 2) Perform an upgrade process in Site Administrator for each project experiencing the issue. 3) Login to Site Administrator 4) Highlight the project 5) Right Click and select Upgrade Project from the menu. 6) Answer the Prompts 7) If a message appears stating the project is already upgraded, select the option to perform the upgrade again.
