AppDynamics integration with bug tracking and service desk

We are using AppDynamics and are trying to figure out how to automatically create incidents (based on application monitoring) to our Service Management system? And is it possible to automatically enter defects for developers in their issue tracking system. For example in Jira?

I'd recommend using already existing action extensions, which can be found at https://www.appdynamics.com/community/ Hopefully, your systems would be there. For example, https://community.appdynamics.com/t5/Knowledge-Base/New-in-4-3-3-Atlassian-Jira-Integration/ta-p/24436 if you are using Jira for defects management.
