I have seen this issue reported with ALM 12.00 and is resolved with the application of patch1. You have installed the ALM 12.00 SP1 pack that rolls the version up to ALM12.01. You will now either need to apply the patch 1 for that build or apply the ALM 12.01 direct install (which supersedes the current patch 1).
I would recommend attempting that and if the issue still is not resolved, then I would elevate the log levels to ''Debug'' and then recreate the issue and capture the commands issued on the back end. I would then de-elevate the log level and open a case with your support provider for further assistance.
