why do QTP test results sometimes show screen capture for errors & sometimes not?

When I run an automated script in QTP and it gets errors, I look at the results file. When I drill down and select each error, sometimes it shows me a screen capture app under test associated with that error, and sometimes it does not. I do not see any pattern so far. What am I missing, or is this a "quirk" of QTP?

This could be due to the screen saver running or if you are using it remote and RDP being minimized. Please provide more details like the screen capture is blank or black and the version of QTP.

I have found in the past if you do an image capture on error and the object does not exist then QTP can not identify where to take the image capture. I make a recovery scenario with a function to take a screen capture of the whole desktop
