Getting ”Sequence Contains more than one matching element” when trying to save Gui Tests (QTP) in UFT

We have UFT 11.51 Build 1267 and QC11 patch 12. Sometimes after we save new tests in Quality Center, when we try to modify the existing tests we get "Sequence Contains more than one matching element" Error. Has anyone seen this before?

I like the workaround fix

Could the be due to child action having same name?

I have had someone try this work-around, and it worked for them too.
